Bellefaire's Transitional Living Program is an on-campus, structured, group living program designed to serve clients ages 17 and older who are nearing the age of independent living, but who have mental health issues and are deficit in skills required to function in an independent living setting.
Service Description
Therapeutic goals are accomplished through an independent living skills "phase" curriculum, community support program service, and individual counseling. Together, these complementary elements work to empower each client in the following areas: daily living skills, medical and mental health care, vocational training, and employment education.
Clients who have not completed high school or a GED program can attend their community school or Cleveland Heights High School. The Bellefaire School is another option for students requiring more intensive services.
Contact With The Primary Child Care Worker
A child care worker monitors the daily progress of each Transitional Living Program participant. In addition, each client's primary worker meets with him or her at least weekly to review individual goals; independent living-skills development; and program, educational, and employment expectations.
Discharge and Transition Planning
During the intake process, a discharge plan is formulated with the client and case manager. The Transitional Living Treatment Teams work closely with placement agencies on individual discharge and transition plans. Many clients progress to the Bellefaire JCB Independent Living Program.
Supervisory support is available 24 hours a day. In addition, Bellefaire 's case managers and therapists provide varying levels of individual and family counseling as required to achieve the treatment goals of each client. Clients receive routine medical and dental care from the Bellefaire JCB health clinic.
